The clinic will be held at FMS on 5/17/22 from 9:00 am - 3:00 pm for incoming 8th-grade students and at Farmington High School on 6/10/2022 from 9:00 am- 3:00 pm for 9th- 12th graders.
If you would like your student to participate in one of these clinics, please complete the attached registration form and return forms to the school. (Please include a copy of the student’s insurance cards if applicable.)
